(Jamaica Gleaner) The Health Ministry has reassigned several employees of the Western Regional Health Authority who worked at the COVID vaccination site from which ten doses have gone missing.

In a release late Tuesday evening, the ministry said a vial with ten doses of the AstraZeneca vaccines went missing from a vaccination site at the Cornwall Regional Hospital in St James on Monday.

The vial was discovered missing after standard checks were carried out during the vaccination process.

The ministry said an investigation is now underway including a review of CCTV footage and the policies and procedures at the site.

Meanwhile, the Health Ministry says it is now reviewing reports that six doses of the AstraZeneca vaccines were accidentally damaged at the Lucea Health Centre in Hanover.

It happened when a vial fell from the hand of a worker who was returning it to a cold storage container.

The ministry said it is also checking into reports that eight doses of the vaccine broke after falling from a cold storage container that was on a table that collapsed.

This happened at a site in Westmoreland.

The ministry undertakes periodic audits of personnel, process flow and usage of vaccines and promises to make public the outcome of its investigations.
